Church Administrations

In consideration of being allowed to particapate as a (Church body) our services or financial support with HH4C to help those in need, we hereby acknowledge that there are certain risks of injury involved, and we knowingly and freely assume all such risks and assume full responsibility for our participation. To the extent allowed by law, we agree to indemnify and hold harmless HH4C, its officers, employees, agents, representatives and volunteers, of all liabilities and all loss or damage to person or property which may occur or be incident to our involvement or participation.

We recognize that as a(Church body) financially supporting or particapating with HH4C, a Washington non-profit corporation (HH4C), we may have access to confidential information concerning HH4C, its clients, employees volunteers or representatives. In consideration of any (Church supporting role) or status with HH4C, we agree we will not at any time, during or after (Church support) for HH4C, divulge or reveal to any person, firm, or corporation, any information (including, but not limited to, personal or financial information or customer lists), directly or indirectly, which might in any way be used to injure or interfere with the business or ministry of HH4C, its clients, employees, volunteers or representatives from HH4C or to cause discontent or dissatisfaction among any such persons.

BY SUBMITTING THE FORM BELOW, WE UNDERSTAND THAT WE ARE INDICATING OUR AGREEMENT WITH ALL THE TERMS OF ABOVE SECTIONS. PLEASE LIST ALL BOARD MEMBERS WITHIN YOUR CHURCH BODY TO SHOW THAT THEY ARE IN AGREEMENT WITH THE STATEMENTS ABOVE. YOU MAY DO SO  IN THE COMMENT SECTION BELOW.
